Facility Waiver & Release
In consideration of using the Prospects Sports Academy facility at 3804 S. Elm Pl., Suite A, Broken Arrow, OK 74011, I understand that baseball, softball, and athletic training involve risk of injury, including serious injury. I voluntarily assume those risks for myself and, if signing for a minor, for that athlete.
I release Prospects Sports Academy, its owners, coaches, and staff from claims arising from ordinary participation, except for claims caused by gross negligence or willful misconduct. I confirm I am legally authorized to sign, and that emergency contact information is accurate.
This waiver covers Prospects activity for one year from the signature date. Questions: (918) 922-8114.
If the participant joins a Prospects Sports Academy team, I authorize Prospects Sports Academy to display the participant’s full name and jersey number on the team’s publicly accessible online roster. Team records and aggregate team statistics are also public. Individual player statistics are restricted to verified members, parents and coaches of that team and authorized administrators. This consent does not authorize publication of contact information, date of birth, medical information, private documents, or photos.
